Bios:
Toyin Ajayi
Toyin Ajayi, MD, MPhil, is Chief Medical Officer at Commonwealth Care Alliance (CCA), a Massachusetts nonprofit health plan and integrated care delivery system created to provide high quality care to people with complex health needs. CCA manages more than 800 million dollars annually to care for approximately 20,000 individuals who, by virtue of being over 65 and low income or younger than 65, disabled and low income, are dually eligible for Medicare and Medicaid benefits. CCA’s members receive a tailored array of coordinated services (primary medical, behavioral, social, and long term supports), managed by interdisciplinary care teams, and grounded in patient centered care decisions determined by the specific needs of individual members. As Chief Medical Officer, Dr. Ajayi directs clinical programs and oversees clinical care delivery, care management, and clinical operations. CCA’s portfolio of innovative programs includes integrated home-based primary care and enhancement services; community respite facilities providing acute psychiatric crisis services; a nationally-recognized end-of-life program; a comprehensive care transitions and post-acute care program; and a groundbreaking community medicine pilot program in partnership with the Massachusetts Department of Public Health and EasCare Ambulance Company that deploys specially trained paramedics to patients’ homes to evaluate and provide appropriate treatment for acute complaints as an alternative to emergency room or hospital visits.

Dr. Ajayi is a board-certified family physician and maintains an active clinical practice, serving as primary care physician to women over 65 years of age who are homeless or marginally housed. She also serves as a Boston Medical Center (BMC) hospitalist, caring for acutely sick patients admitted to CCA’s shared inpatient service unit at BMC.

Prior to joining CCA in 2013, Dr. Ajayi was an attending physician at BMC’s Department of Family Medicine – where she was awarded the Department’s Faculty Teaching Award in 2013 – and a clinical instructor at Boston University’s School of Public Health. Dr. Ajayi received her undergraduate degree from Stanford University; an MPhil from the University of Cambridge; and her medical degree, with Distinction in Clinical Practice, from King’s College London School of Medicine.

Alexis Bernstein
Alexis is the Senior Director of Client Services with Wellframe, a mobile care management company. In this role, she oversees the operational and strategic needs of Wellframe’s customer base. Previously, she was an initial team member of and served as a Director of Strategy and Operations in Aetna’s Accountable Care Solutions team. Previously, Alexis was a Strategic Planner with ACS and with Aetna’s IT subsidiary, ActiveHealth Management, a care management company. In these roles Alexis both managed client relationships and built organizational infrastructure for these nascent businesses. Earlier in her career, Alexis worked in consulting and research at Booz Allen Hamilton and the Advisory Board Company. Alexis earned an MBA from Wharton with a focus in Health Care Management and her bachelors in the History of Science from Harvard University.

Rebecca Bialecki
Long-time community leader and former Athol Hospital and Heywood Healthcare Trustee, Rebecca Bialecki, joined the Heywood Healthcare Senior Leadership team in 2014 as its Vice President for Community Health Initiatives and Chief Agent of Change. She holds a doctorate in clinical psychology.
Well-known in the North Quabbin region as a visionary and voice for under-served and at-risk populations, Rebecca brings her skills to the Heywood team as she leads strategic projects focused on improving the overall health of our region’s families. Key roles include leading the development of the Quabbin Retreat in Petersham, coordinating the health system’s multi-million dollar CHART grant projects and helping spearhead Athol Hospital’s planning, fundraising and implementation of a new Emergency Department and medical office building. She oversees the Telemedicine programs across the system developing teleneurology, and now offering telepsych both in-patient and within local partner school systems.

Kathleen A. Connolly
Kathleen Connolly is the Director for Strategic Investment at the Health Policy Commission. In this role, Kathleen will be managing the CHART Investment Program and the Health Care Innovation Investments. Prior to joining the HPC, Kathleen worked at CVS Health, where she was the Director of Population Health, Health System Alliances. She also previously served as a Program Director for Population Health Management at Partners HealthCare. Kathleen graduated Magna Cum Laude from Connecticut College before pursuing her Master’s degree in Social Work at the University of Connecticut. She is a Licensed Independent Clinical Social Worker in Massachusetts.
